Additional Product Terms
- When any test (drug test, alcohol test, physical or any service) is scheduled or purchased and the test registration and/or donor pass is issued, the test is non-refundable. Tests or services may be rescheduled or a credit will be provided for future testing and/or services. If your donor passport expires we can reschedule your test or service.
- Test registration / donor passes are issued via email Monday – Friday 7:30am to 8:00pm PST and Saturday 9:00am to 5:00pm PST. Online registrations scheduled after specified hours or on Sunday will be sent the following business morning.
- In the event a test or service is not available at a preferred location an alternate test center will be selected closest to the preferred test location. Not all locations will offer the same services.
- In most cases test results are available within three business days for urine drug tests and five business days for hair follicle drug tests; however laboratory confirmation testing may take longer. Hair follicle drug tests require a minimum of 2 inches of hair.
- DOT Consortium Membership includes certificate of compliance. 5 or more employees enrolled into the Consortium will qualify for discounted pricing.
- A DOT (Department Of Transportation) drug test requires a Federal chain of custody form which can be supplied electronically or by paper. RAD Testing & Mobile Collection can provide either.
- Rapid Drug Tests are not available in all testing locations or in all states. Each state has different laws about drug testing requirements.
- If a Rapid Drug Test is ordered and not available or not allowed in a specific state the drug test will be changed to a standard laboratory test. The test will be shipped to the lab for all testing.
- Any rapid result test which is non-negative will be sent to the lab for confirmation and MRO (Medical Review Officer or doctor) review. Pricing includes MRO review and confirmation testing.
- Invoicing to corporations or employers for account receivable can be made available upon approval. Contact RAD Testing to qualify and for specific details.
- Out of network testing may require an additional fee or payment directly to the out-of-network facility.
- Discounts can be made available for multiple test purchases. Inquire before purchase.
